Short answer
Restricted areas near Nepal's northern border need a special trekking permit from the Department of Immigration, arranged through a registered trekking agency, on top of the usual park or conservation fee. Upper Mustang and Upper Dolpo cost USD 50 per person per day. Manaslu costs USD 100 for the first week from September to November and USD 75 the rest of the year, then a daily rate. You must stay on the permitted route and show the permit when asked.

Common problems
Did the Upper Mustang fee change?
Yes. It used to be USD 500 for the first ten days. The Nepal Tourism Board now lists USD 50 per person per day, so short trips cost less and ten days still cost USD 500.
Can I apply myself?
Agencies apply on the Department of Immigration's online system. Go through a registered trekking agency.
Do I still pay the conservation area fee?
Yes. For example, Upper Mustang and Manaslu also need the Annapurna or Manaslu conservation area permit.
